Chronic Disease

Chronic conditions require coordinated, proactive care — not just reactive treatment when symptoms flare. Dr Adams works closely with patients to develop care plans that address both the medical and lifestyle aspects of their condition, and reviews them regularly to ensure they stay relevant.

Dr Adams' Approach

Proactive, Not Reactive

Regular reviews are scheduled before problems arise — not only when something goes wrong. The aim is to stay ahead of complications, not catch up to them.

The Whole Picture

Chronic conditions rarely exist in isolation. Dr Adams considers how conditions interact, how medications affect each other, and how lifestyle factors into long-term outcomes.

Coordinated Care

Where specialists, allied health, or other services are involved, Dr Adams coordinates that care — so you have a single point of contact who understands the full picture.

Medicare-Supported Care Plans

Patients with chronic conditions may be eligible for a GP Chronic Condition Management Plan (GPCCMP) a Medicare-funded plan that provides structured care and subsidised access to allied health services such as physiotherapy, dietetics, and psychology. Ask Dr Adams whether a care plan is right for you.
