Medicare-Supported Care Plans
Patients with chronic conditions may be eligible for a GP Chronic Condition Management Plan (GPCCMP) a Medicare-funded plan that provides structured care and subsidised access to allied health services such as physiotherapy, dietetics, and psychology. Ask Dr Adams whether a care plan is right for you.
